In order to serve you more promptly and efficiently with your ongoing medications, we ask that you follow the policy below to ensure no break in continuity of your care.
Ongoing medications do require follow-up care with the physicians and/or ongoing lab monitoring . Prescriptions will not be filled if maintenance appointments are not kept. Please plan ahead for all refills, written or otherwise.
We require at least 72 hours notice for all referrals. Please be prepared with all necessary information - think who, what, when, where, and why. Lastly, remember to plan ahead and check your insurance plan benefits.
Please be familiar with the requirements of your insurance policy. Review the terms and agreements as set forth in your insurance manual. Please assist us by knowing your plan benefits, including co-pay, referrals, pre-certification, covered facilities, and services information. We file both primary and secondary claims for our patients.
Patients are required to pay all applicable co-pays and deductibles at the time of service. Failure to do so will result in an additional $15.00 processing fee. Patients are also responsible for any non-covered service fees performed at the time of service. You will be informed of these services and asked to sign a waiver stating the same.

*Please be aware that we cannot treat any work-related conditions. Please check with your employer for their recommended worker’s care facility.
*We also cannot bill any third party carriers for motor vehicle or personal injury cases. The patient is responsible for payment at the time of service. A receipt can be issued for reimbursement.
